These questions will form part of the S1 Primary Health Care Screen carried out by Nurses in the First Night Centre on day one in custody.

Before coming into prison was the client receiving any support with daily tasks?(Required)
Was the client in receipt of Disability Living Allowance (DLA), Personal Independence Payment (PiP) or attendance allowance prior to custody?(Required)
Was the client paying for part or all of their care?(Required)
Does the client have a physical, sensory (hearing/visual), or learning disability?(Required)
Has the client previously attended a Special Education Needs School?(Required)
Before the client came into prison did they have any specialist equipment or adaptation where they lived?(Required)
Does the client remember to take their medication, or require support to take medication on time?(Required)
Which daily activities does the client have difficulty completing?
Is the client able to wash and dress themselves?(Required)
Are there any risk identified to client?(Required)
Are there any skin integrity/tissue viability issues?(Required)
Have these been recorded, reported and referrals made to specialist services?(Required)
